About Home & Community Services

Background & History

Since July 1, 1998, Home and Community Services, Inc. (formerly UCP Home & Community Services)  has operated as an in-home care and community based services contractor/provider.  We are an experienced  provider of Medicaid Waiver and non-waiver services through the Pennsylvania DPW Office of Long Term Living, (formerly the Office of Social Programs), the Pennsylvania Department of Aging, and other public programs that fund in-home and community based services.

To address the growing need for home and community based services, Home & Community Services was incorporated in 1998 and programs and services were expanded. The expansion included the addition of various programs and funding sources to broaden the consumer population served by the agency. The development of new programs enabled the agency to provide a continuum of services from children to adults over the age of 60.

The agency changed its names on July 1, 2009 from UCP Home and Community Services, Inc. to simply Home & Community Services, Inc.  

We initially began providing services as a program under United Cerebral Palsy of Southwestern PA, now Pathways of Southwestern Pennsylvania, Inc. in 1993. 

 

Mission

The mission of Home & Community Services is to affect positively the quality of life and to promote advancements for persons with disabilities. Home & Community Services is committed to providing the highest quality of service to consumers and their families.
 

 

Philosophy

It is the philosophy of Home & Community Services:

  • to continually strive to meet the current and future needs of persons with disabilities.
     
  • to emphasize the transition of people into higher levels of employment while maintaining enrollment in the program.
     
  • to continue to provide individualized support services and training to persons with disabilities and their families, while providing alternatives to institutionalization and increasing community integration.
     
  • to assist persons with physical disabilities with purchasing, arranging, coordinating, implementing, monitoring, and evaluating services.
     
  • to maintain a positive relationship with other disability organizations to aid in improving the existing quality of our services.
      
  • to assure that governance procedures comply with all legal statues, satisfy accreditation/affiliation requirements, and conform to funding sources guidelines.
